How to Make a Family Medical Binder (Step by Step, from a Physician)

Cómo hacer una carpeta médica familiar, paso a paso

Written by a family medicine physician & certified medical interpreter · Ambos Health

A medical binder is the single most useful thing a family can bring to an emergency room. It doesn't need to be fancy. It needs to be complete, current and findable.

What you need

The 8 sections that matter

  1. Emergency — a one-page summary per person: allergies, conditions, medicines, emergency contacts, doctor, pharmacy, insurance, advance directive location.
  2. Health profile — conditions, surgeries and hospital stays, allergies (with what actually happened), family history, vaccines, screenings.
  3. Medications — current list, weekly checklist, supplements, and stopped medicines with why they were stopped.
  4. Appointments — calendar, visit prep sheets, visit notes, symptom diary, lab results, referrals.
  5. Trackers — blood pressure, blood sugar, weight, fever logs for kids.
  6. Insurance & costs — plan details, HSA/FSA receipts, a bill-and-EOB tracker.
  7. Caregiving & planning — where important documents are kept, a care handoff sheet, hospital stay log.
  8. Reference — glossary and phrases if your family uses more than one language.

Start here (10 minutes)

Fill in the Emergency Health Summary for the person you worry about most. Make three copies: fridge, wallet or purse, car. Take a photo on your phone. Everything else can be filled in over a weekend.

Privacy

Write where sensitive documents live, not the numbers. Never write a Social Security number or passwords in the binder.

Keep it current

Update the medication list after every visit. Review the whole binder twice a year — birthdays and New Year are easy reminders.
